Engineers pasted credentials into private channels during incidents. It was faster than the alternative, and it produced the same audit finding every cycle. Policy alone had not changed the behaviour.
The approach
The secrets manager made referencing a credential quicker than pasting one. Row-level security applied to every query including the AI layer's, so incident summaries never exposed anything the reader could not already open.
